David Peirce Physiotherapy may collect identification details – including name, date of birth, contact numbers, email address, and relevant health information. This information will not be passed on to outside parties. We understand that this information is highly sensitive and is only collected with your consent. This information is collected through new client forms or during a consultation and assists us in providing you with personalised services.
Please keep your information up to date so that we can continue to provide you with personalised service and ensure that you receive information on our products and services.
Our sign-on process gives you the option of subscribing to receive newsletters via email. These emails will in no way disclose your unique health characteristics. At the time you sign up for our email newsletters or any time thereafter, you can choose to opt-out of receiving additional promotional emails from us.

INFORMED CONSENT
The physiotherapists in this practice will discuss your condition and options for treatment with you so that you are appropriately informed and can make decisions relating to treatment. You may choose to consent to or refuse any form of treatment for any reason including religious or personal grounds. Once you have a given consent you may withdraw that consent at any time.
Questions of a personal nature
Your physiotherapist may ask personal questions relating to your pain or injury and how your injury impacts on your day-to-day life. These quaetions will only be aske if relevant to your particular pain or injury and help us to help you. It is your choice what information you choose to provide.
Removal of clothing
To adequately assess and treat your condition, your Physiotherapist may ask you to remove clothing if appropriate and necessary. You may refuse to do so for any reason.
Physical contact
During the examination, assessment and treatment it may be necessary for your physiotherapists to make physical contact. Your physiotherapist will ask your permission before making physical contact with you in any way.
Risks related to treatment
As with any form of therapeutic treatment, there are risks and benefits. The physiotherapist will discuss any foreseeable risks with you prior to administering treatment.
Children and minors
Consent from a custodial parent or guardian is required to treat a minor (anyone under 18 years of age) Medical information and consultation details can be withheld from parents / guardians if a minor is able to make their own medical decisions where they have sufficient understanding and intelligence to fully understand the treatment.
Substituted Consent
Where a person is incapable of understanding the risks and benefits of treatment consent may be provided by another person legally authorised to provide such consent. Evidence of legal authorisation is required in such circumstances.
